Call Assists Data Subject Request Form

The Call Assist Data Protection Portal has been created to assist individuals with exercising their individual rights in relation to the personal data that Call Assist retains in line with General Data Protection Regulation (GDPR).

Our Data Subject Request Form is available to be completed via the Call Assist Data Protection Portal using the ‘Online Request’ option or by selecting the ‘Download Form’ option to be completed manually.

Type Of Request

In line with GDPR, Call Assist provides you with the option to exercise the following Individual Rights in relation to your personal data:

  • Right of Access
  • Right of Rectification
  • Right to Erasure
  • Right to Restrict Processing
  • Right to Data Portability
  • Right to Object
  • Rights in relation to Automated Decision Making and Profiling

What data do we ask for and why?

At Call Assist we make Data Security an integral part of our daily operation. For this reason we will require you to provide us with the following information to process your request:

  1. Forename
  2. Surname
  3. Email Address or Full Postal Address
  4. Identification - used solely as Proof of Identity

In order for your request to be fulfilled, we will require you to provide Proof of Identity. Acceptable forms of Identification include:

  • Passport
  • UK Drivers Licence
  • EU Identity Card
  • Council Tax Bill
  • Credit Card Statement
  • Pension Book
  • Bank Account Statement

Your identification can be provided via the Online Data Subject Request Form or a copy of your identification can be provided with the downloadable Data Subject Request form when submitted.

Your identification will be used for the sole purpose of proof of identity and retained using our secure portal. Once the verification process has been completed your identification will be erased from our system within 72 hours.

The protection of your personal data is a priority to Call Assist and for the purpose data security; we will be unable to fulfil your request until we can verify that the request has been made by the individual to which the data relates.

Any additional information that you wish to provide can be done so voluntarily and is not a requirement in order for your request to be processed.

Your right to complain to the Supervisory Authority (ICO)

If you are concerned about how Call Assist processes your personal data, you have the right to complain to the Information Commissioner’s Office (ICO). The ICO anticipates that you will raise your concern with us in the first instance.
